        Medicalising borders

        Selection, containment and quarantine since 1800

        The research of pandemics, epidemics, and pathogens like COVID-19 reaches far beyond the scope of biomedicine. It is not only an objective for the health, political and social sciences, but epidemics and pandemics are a matter of geography: foci and vectors of communicable diseases continue to test the efficacy of medical control at state borders. This volume illuminates these issues from various disciplinary viewpoints. It starts by exploring historical models of quarantine, spatial isolation and detention as precautionary means against the dissemination of disease and contagion by border crossers, migrants and refugees. Besides the patterns of prejudice with which these groups are confronted, the book also deals with various kinds of fear of contamination from outside of the nation state. The contributors address the implementation of medical techniques at state borders in the twentieth and early twenty-first centuries, as well as the presently practiced measures of medical and biometric screening of migrants and refugees.         Summary of a Workshop on the Technology, Policy, and Cultural Dimensions of Biometric Systems

        Biometricsâ€"the use of physiological and behavioral characteristics for identification purposesâ€"has been promoted as a way to enhance security and identification efficiency. There are questions, however, about, among other issues, the effectiveness of biometric security measures, usability, and the social impacts of biometric technologies. To address these and other important questions, the NRC was asked by DARPA, the DHS, and the CIA to undertake a comprehensive assessment of biometrics that examines current capabilities, future possibilities, and the role of the government in their developments. As a first step, a workshop was held at which a variety of views about biometric technologies and systems were presented. This report presents a summary of the workshop’s five panels: scientific and technical challenges; measurement, statistics, testing, and evaluation; legislative, policy, human, and cultural factors; scenarios and applications; and technical and policy aspects of information sharing. The results of this workshop coupled with other information will form the basis of the study’s final report.

        Biometric Recognition

        Biometric recognition--the automated recognition of individuals based on their behavioral and biological characteristic--is promoted as a way to help identify terrorists, provide better control of access to physical facilities and financial accounts, and increase the efficiency of access to services and their utilization. Biometric recognition has been applied to identification of criminals, patient tracking in medical informatics, and the personalization of social services, among other things. In spite of substantial effort, however, there remain unresolved questions about the effectiveness and management of systems for biometric recognition, as well as the appropriateness and societal impact of their use. Moreover, the general public has been exposed to biometrics largely as high-technology gadgets in spy thrillers or as fear-instilling instruments of state or corporate surveillance in speculative fiction. Now, as biometric technologies appear poised for broader use, increased concerns about national security and the tracking of individuals as they cross borders have caused passports, visas, and border-crossing records to be linked to biometric data. A focus on fighting insurgencies and terrorism has led to the military deployment of biometric tools to enable recognition of individuals as friend or foe. Commercially, finger-imaging sensors, whose cost and physical size have been reduced, now appear on many laptop personal computers, handheld devices, mobile phones, and other consumer devices. Biometric Recognition: Challenges and Opportunities addresses the issues surrounding broader implementation of this technology, making two main points: first, biometric recognition systems are incredibly complex, and need to be addressed as such. Second, biometric recognition is an inherently probabilistic endeavor. Consequently, even when the technology and the system in which it is embedded are behaving as designed, there is inevitable uncertainty and risk of error. This book elaborates on these themes in detail to provide policy makers, developers, and researchers a comprehensive assessment of biometric recognition that examines current capabilities, future possibilities, and the role of government in technology and system development.

        Perspectives on Biomarker and Surrogate Endpoint Evaluation

        In 2010 the Institute of Medicine (IOM) recommended a framework for the evaluation of biomarkers in the chronic disease setting. Published in the book Evaluation of Biomarkers and Surrogate Endpoints in Chronic Disease, the framework is intended to bring consistency and transparency to the previously disparate process of biomarker evaluation. Following the book's release, the IOM convened a 2-day discussion forum in Washington, DC, in order to provide an opportunity for stakeholders to learn about, react to, and discuss the book. Presentations reviewed the authoring committee's work process, recommendations, and provided perspectives on the book from the point of view of participants. Thomas Fleming, professor of biostatistics and statistics at the University of Washington, gave a keynote presentation on the critical issues in the validation of surrogate endpoints, a specific use of a biomarker. The present volume recounts the discussion forum proceedings, focusing in turn on each represented sector. A summary of Dr. Fleming's presentation then sets the committee's recommendations within the context of biomarker utilization. Lastly, this summary examines the main themes raised by stakeholders, and the challenges and opportunities presented to stakeholders by the book's recommendations.

        Monitoring HIV Care in the United States

        A Strategy for Generating National Estimates of HIV Care and Coverage

        In September 2010, the White House Office of National AIDS Policy commissioned an Institute of Medicine (IOM) committee to respond to a two-part statement of task concerning how to monitor care for people with HIV. The IOM convened a committee of 17 members with expertise in HIV clinical care and supportive services, epidemiology, biostatistics, health policy, and other areas to respond to this task. The committee's first report, Monitoring HIV Care in the United States: Indicators and Data Systems, was released in March 2012. The report identified 14 core indicators of clinical HIV care and mental health, substance abuse, and supportive services for use by the Department of Health and Human Services (HHS) to monitor the impact of the National HIV/AIDS Strategy (NHAS) and the Patient Protection and Affordable Care Act (ACA) on improvements in HIV care and identified sources of data to estimate the indicators. The report also addressed a series of questions related to the collection, analysis, and dissemination of data necessary to estimate the indicators. In this second report, Monitoring HIV Care in the United States: A Strategy for Generating National Estimates of HIV Care and Coverage, the committee addresses how to obtain national estimates that characterize the health care of people with HIV within the context of the ACA, both before 2014 and after 2014, when key provisions of the ACA will be implemented. This report focuses on how to monitor the anticipated changes in health care coverage, service utilization, and quality of care for people with HIV within the context of the ACA.

        Review of the U.S. Navy Environmental Health Center's Health-Hazard Assessment Process

        A large number of chemicals are used on land at shore facilities, in the air in combat and reconnaissance aircraft, on seas around the world in surface vessels, and in submarine vessels by the navy and marine corps. Although the chemicals used are for the large part harmless, there is a significant amount of chemicals in use that can be health hazards during specific exposure circumstances. The Navy Environmental Health Center (NEHC) is primarily tasked with assessing these hazards. The NEHC completes its tasks by reviewing toxicological and related data and preparing health-hazard assessments (HHAs) for the different chemicals. Since the NEHC is continually asked to develop these HHAs, the National Research Council (NRC) was asked to assess independently the validity and effectiveness of NEHC's HHA process, in order to determine whether the process as implemented provides the Navy with the best, comprehensive, and defensible evaluations of health hazards and to identify any elements that might require improvement. The task was assigned to the Board on Environmental Studies and Toxicology's Committee on Toxicology's (COT's) Subcommittee on Toxicological hazard and Risk Assessment. Review of the U.S. Navy Environmental Health Center's Health-Hazard Assessment Process presents the subcommittee's report. The report is the work of expertise in general toxicology, inhalation toxicology, epidemiology, neurotoxicology, immunotoxicology, reproductive and developmental toxicology, pharmacology, medicine, risk assessment, and biostatistics. It is based on its review of documents provided by NEHC, presentations by NEHC personnel, and site visits to NEHC in Norfolk, Virginia and an aircraft carrier in San Diego, California.

        Toxicological Risks of Selected Flame-Retardant Chemicals

        Ignition of upholstered furniture by small open flames from matches, cigarette lighters, and candles is one of the leading causes of residential-fire deaths in the United States. These fires accounted for about 16% of civilian fire deaths in 1996. On average, each year since 1990, about 90 deaths (primarily of children), 440 injuries, and property losses amounting to 50 million dollars have resulted from fires caused by the ignition of upholstered furniture by small open flames. Certain commercial seating products (such as aircraft and bus seats) are subject to flammability standards and sometimes incorporate FR-treated upholstery cover materials, but there is no federal-government requirement for residential upholstered furniture, and it is generally not treated with FR chemicals. It is estimated that less than 0.2% of all U.S. residential upholstery fabric is treated with flame-retardant (FR) chemicals. The Consumer Product Safety Act of 1972 created the U.S. Consumer Product Safety Commission (CPSC) as an independent federal regulatory agency whose mission is to protect the public from unreasonable risks of injury and death associated with consumer products. CPSC also administers the Flammable Fabrics Act, under which it regulates flammability hazards and the Federal Hazardous Substances Act (FHSA), which regulates hazardous substances including chemicals. In 1993, the National Association of State Fire Marshals petitioned CPSC to issue a performance-based flammability standard for upholstered furniture to reduce the risk of residential fires. The Commission granted that portion of the petition relating to small open flame ignition risks. In response to concerns regarding the safety of FR chemicals, Congress, in the fiscal year 1999 appropriations report for CPSC, requested that the National Research Council conduct an independent study of the health risks to consumers posed by exposure to FR chemicals that are likely to be used in residential upholstered furniture to meet a CPSC standard. The National Research Council assigned the project to the Committee on Toxicology (COT) of the Commission on Life Sciences' Board on Environmental Studies and Toxicology. COT convened the Subcommittee on Flame-Retardant Chemicals, which prepared this report. Subcommittee members were chosen for their recognized expertise in toxicology, pharmacology, epidemiology, chemistry, exposure assessment, risk assessment, and biostatistics. Toxicological Risks of Selected Flame-Retardant Chemicals is organized into 18 chapters and two appendices. Chapter 2 describes the risk assessment process used by the subcommittee in determining the risk associated with potential exposure to the various FR chemicals.         Review of the Department of Labor's Site Exposure Matrix Database

        Beginning with the development of the atomic bomb during World War II, the United States continued to build nuclear weapons throughout the Cold War. Thousands of people mined and milled uranium, conducted research on nuclear warfare, or worked in nuclear munitions factories around the country from the 1940s through the 1980s. Such work continues today, albeit to a smaller extent. The Department of Energy (DOE) is now responsible for overseeing those sites and facilities, many of which were, and continue to be, run by government contractors. The materials used at those sites were varied and ranged from the benign to the toxic and highly radioactive. Workers at DOE facilities often did not know the identity of the materials with which they worked and often were unaware of health risks related to their use. In many instances, the work was considered top secret, and employees were cautioned not to reveal any work-related information to family or others. Workers could be exposed to both radioactive and nonradioactive toxic substances for weeks or even years. Consequently, some of the workers have developed health problems and continue to have concerns about potential health effects of their exposures to occupational hazards during their employment in the nuclear weapons industry. In response to the concerns expressed by workers and their representatives, DOL asked the Institute of Medicine (IOM) to review the SEM database and its use of a particular database, Haz-Map, as the source of its toxic substance-occupational disease links. Accordingly, this IOM consensus report reflects careful consideration of its charge by the committee, and describes the strengths and shortcomings of both. To complete its task, IOM formed an ad hoc committee of experts in occupational medicine, toxicology, epidemiology, industrial hygiene, public health, and biostatistics to conduct an 18-month study to review the scientific rigor of the SEM database. The committee held two public meetings at which it heard from DOL Division of Energy Employee Occupational Illness Compensation (DEEOIC) representatives, the DOL contractor that developed the SEM database, the developer of the Haz-Map database, DOE worker advocacy groups, and several individual workers. The committee also submitted written questions to DOL to seek clarification of specific issues and received written responses from DEEOIC. The committee's report considers both the strengths and weaknesses of the SEM and the Haz-Map databases, recognizing that the latter was developed first and for a different purpose. The committee then discusses its findings and recommends improvements that could be made in both databases with a focus on enhancing the usability of SEM for both DOL claims examiners and for former DOE workers and their representatives. Review of the Department of Labor's Site Exposure Matrix Database summarizes the committee's findings.
